I have Blue Cross / Blue Shield in Massachusetts and it was covered . . . in fact all of our fertility treatments from testing to treatments including IUI and IVF were covered with just our co-pays.
If your employer is in MA / your insurance is through Massachusetts branch of Blue Cross, you should be fine. MA is one of 15 states to have legislation requiring fertility coverage in most situations. You can check out the Resolve website for more info:

ask your dr to bill it as treatment for endomitriosis, not infertility. you have a better chance of coverage that way.

I have blue cross Montana and mine was also covered!! you might just email or call them and find out!!
